Authorization and Acknowledgment Statement
I hereby authorize Wanderlust Travel, LLC to charge my credit card, as referenced above, for the amount indicated. I acknowledge and accept all Terms and Conditions associated with this booking, including but not limited to cancellation policies, applicable penalties, and the availability of travel insurance.
By submitting this form, I, the cardholder identified in the credit card section above, authorize Wanderlust Travel, LLC to make payments on my behalf to the designated travel providers. Payments will be automatically processed for the fixed dollar amount you have selected and authorized. Please note that it may take 24–48 hours for the charge to be reflected on your reservation. I understand that I am bound by the terms and conditions of this booking.
I acknowledge that certain travel arrangements may carry non-refundable cancellation penalties, and I accept this as part of the booking process.
I agree to carefully review all communications received from Wanderlust Travel, LLC via email and to take note of any applicable restrictions or conditions.
I further acknowledge that Wanderlust Travel, LLC strongly recommends the purchase of travel insurance to help safeguard my travel investment.

By signing below, I agree to receive all communications, disclosures, and documents related to this transaction electronically. I understand that I may withdraw my consent at any time by contacting Wanderlust Travel, LLC, though doing so may impact the ability to complete this booking or receive important updates.
I, the undersigned cardholder, certify that the information provided in this form is accurate and complete. I affirm that I am authorized to approve charges on the credit card provided. In the event of a discrepancy on my credit card statement, I agree to notify Wanderlust Travel, LLC within seven (7) business days of receiving the statement or immediately upon discovering the issue.
If using a debit card with a daily spending limit, I understand it is my responsibility to contact my bank to authorize this transaction. Should the bank require the vendor's name and I am unsure of it, I will contact Wanderlust Travel, LLC for clarification.
